1. Is there procedures / work instructions are prepared? Is all are documented?

2. Is there procedures / work instructions are deployed at confined space entry area? People trained for procedure / work instructions? Whether effectiveness of training checked? Evidence?

3. Is there permission is granted for the work in confined space? A person is permitted for work in confined space?

4. Persons trained for the work in confined spaces

5. Is there permit form, permit requirements is identified properly?

6. Is there any schedule for the training annually? Is there schedule are indicate re-training for confirmed space work? Is there annual calendar for training cover new people to involve for training for confined space entry?

7. Check visually confined space, is there purged with fresh air?

8. Is there confined space entry program is developed? All are documented?

9. Lifelines, harness apparatus and respirators in place?

10. Are there any unsafe conditions / unsafe working behaviors / unsafe activities or material found during inspection?
